For patients taking diuretic, a low FeNa can be helpful as it shows the patient is in a sodium avid state. If FeNa is elevated it is not useful.

Tolvaptan is contraindicated in hypovolemic hyponatremia as it could precipitate shock state.

Vaptans are approved for use in euvolemic/hypervolemic causes of hyponatremia which could not be manages with other strategies.

If Tolvaptan is used for greater than 1 month, there is an increased risk for drug induced hepatitis.

For euvolemic hyponatremia, can try salt tabs or purified urea prior to trying vaptans.
