Candida esophagitis
- One of the most common opportunistic infections in AIDS
- Occurs rapidly when CD4 count < 200
- Other populations: solid organ transplant, stem cell transplant, CVID
- Not commonly seen in patients on chronic steroids
- Rare cause of candidemia
HIV testing
- Antigen/antibody includes p24 antigen, which narrows window period between infection and diagnosis
- Very high sensitivity (> 99%)!
- Geenius test delineates between HIV 1 and HIV 2 (HIV 1 much more common in the United States)
Initial therapies for HIV infection
- Think of 2 NRTIs as backbone of regimen
- Can include an NNRTI, boosted protease inhibitor, or integrase inhibitor
- Integrase inhibitor resistance almost non-existent!
