Management Pearls
- S. Aureus have a high proprensity for metastatic disease so must be vigillant, especially if cultures are persistently positive. High rates of endocarditis associated with peripheral IV infections. S. Aureus can also form biofilms
- MRSA bacteremia is likely from carriage, and although nasal swabs are great, there are other sites of potential colonization
- Firstline therapy for MRSA is Vancomycin, but there are many mechanisms for resistance. Check MIC as most common mechanism is called MIC Creep
- 2nd-line is often a combination of ceftaroline and daptomycin
